Programs

RESIDENTIAL SERVICES

The York County Board of Disabilities and Special Needs operates 25 residential programs in York County. There are currently four 8-bed facilities, nineteen 4-bed facilities, one 12-unit apartment complex and an independent living program with 11 participants.

Community Training Homes II (CTH-II)

These four bedroom homes are located throughout the county. These programs provide 24-hour support, skills training and community activity.

Community Residential Care Facilities (CRCF)

These homes house eight residents each and provide 24-hour support, skills training and community activity to consumers requiring a more intensive level of support.

Supervised Living Program II (SLP-II)

Located in Rock Hill, Brown Villas is home to 12 individuals. This 12-unit apartment complex provides 24-hour on-site staff support, skills training, transportation to and from work and other community activities. The SLP-II program serves individuals who possess many of their independent living skills but who still require moderate support.

Supervised Living Program I (SLP-I)

Those participating in the SLP-I program can be found living independently in their own homes throughout the county. These folks possess their independent living skills but require a small degree of support. The program staff does provide skills training and assists with transportation, but they do not provide 24-hour on-site supervision.

VOCATIONAL SERVICES

Horizons Industries

Horizons is the day services component of the York County Board of Disabilities and Special Needs. The Board operates two Work Activity Centers, one in York and the other in Rock Hill. There are varying levels of skills training provided in these settings. Some of the consumers may work on basic social skills and personal care skills, while others work in areas where they are paid to do contract work and their skills training focuses more on increasing their production. The centers are also staging points for mobile work crews and enclaves. Mobile Work Crews currently provide janitorial services and lawn care to companies out in the community. An enclave provides workers and a supervisor to a company so that the work is performed in the contracting company versus the work activity center.

Supported Employment

This program provides assistance in obtaining and sustaining competitive employment. The service provides wages and opportunities for increased community interaction. Job coaches provide on-the-job training, resume building, interviewing training and short-term follow-up to sustain the employment

Individual Rehabilitation Supports

This service provides one-on-one training to consumers in the community. Training goals are focused on the consumer’s areas of interest and needs.

ADDITIONAL COMMUNITY SUPPORTS

Service Coordination

This is the entry point into the South Carolina Department of Disabilities and Special Needs service delivery system. Service Coordinators work with consumers and/or families to determine eligibility. Once eligibility is determined, the service coordinator helps identify needs, develop a plan of services and supports to meet their needs. They assist the consumer and their family access to community resources.

Early Intervention

This is a family focused, in-home service for children (birth – 3). The early interventionist helps the family understand their child’s development. The EI helps the family form and Individualized Family Service Plan (IFSP) and provides service coordination as well as training in accordance to the needs on the plan. The Early Intervention program services children and families eligible for Babynet, South Carolina’s statewide Early Intervention Program.

Respite Services provides short-term, intermittent care while families handle emergencies, run errands or take a vacation. Companion Services provide 1:1 training to consumers in the community based on assessed needs. Recreation Services assist with Special Olympics, exercise and other socialization opportunities. Individual Summer Services provides vouchers for consumers and families to choose summer camps or other activities.
